Lewis & Clark Behavioral Health Services provides advanced support for individuals with severe and persistent mental illness or adults who would benefit from daytime programming. While our Continuous Assistance Rehabilitation & Education (CARE) services are available for individuals in their homes or a community setting our Individualized and Mobile Program of Assertive Community Treatment (IMPACT) provides care 24/7. Community-Based Support Services

Continuous Assistance Rehabilitation & Education (CARE)
Available to those within our seven-county service area.
Individuals with severe and persistent mental illness may need outside support and assistance in the community in order to achieve recovery in their lives. CARE services are provided by a team of case managers who reach out to individuals in their homes or a community setting. Each client is assigned a case manager who, with the active participation of the client, designs an individualized treatment plan outlining the various services which will be provided to reach individualized treatment goals. CARE services are designed to encourage individuals to build up physical, emotional, and intellectual skills to result in developing happy, healthy, and productive lives.

CARE Club
Available to adults in Yankton, South Dakota.
CARE Club’s focus is on the strengths and abilities of members, not on their mental illnesses. CARE Club is a member-driven program and all participation is on a voluntary basis. The core values of CARE Club are to provide a safe and welcoming place for members to come, regardless of current severity of symptoms, to provide opportunities and ownership in the program, and to help facilitate meaningful relationships between members and CARE staff.
CARE Club Seeks to Provide:
- Rehabilitative services in an environment that focuses on peer support and the talents of members.
- Services to promote recovery-oriented goals while emphasizing choice, respect, and opportunities for all members.
- Opportunities for members to confidently participate in the community and reduce the need for more intensive treatment.
- A warm, welcoming place for members despite the current severity of their illness, or their current ability to cope effectively with their illness.
- Opportunities for members to take leadership roles, contribute to the successful day-to-day operations of CARE Club, and develop job skills in a natural work setting.

Available to adults in Yankton, South Dakota.

Individualized and Mobile Program of Assertive Community Treatment (IMPACT)
Available to adults within five miles of Yankton, South Dakota.
IMPACT is tailored for individuals facing severe mental illness, offering intensive community-based services. The program includes a dedicated team of professionals with diverse backgrounds in social work, psychology, nursing, and psychiatry. These services are comprehensive and highly personalized, ensuring round-the-clock availability, 24 hours a day, 7 days a week. Individuals involved with IMPACT must be referred through the Division of Mental Health in Pierre, SD.
Our IMPACT Services Provide
- Assistance with maintaining basic needs such as housing, shopping, laundry, and cleaning
- Coordination and assistance with health care
- Case management that is strength based and co-occurring capable
- 24-hour availability and in-home services
- Monthly psychiatric consultation
- Peer counseling
- Vocational services
- Education about mental illness, medication delivery, and skill development
- Money Management
- Promotion of independence, recovery, and a focus on strengths
- Person centered planning
- Family education and supportive involvement
- Individual and family therapy
