Assisted Living for Mental Wellness
Cedar Village is a Permanent Supportive Housing facility for clients with (1) mental illness and/or substance use diagnoses who (2) meet assisted living level of care and are (3) chronically homeless as evidenced by being homeless continuously for at least one year or on at least four separate occasions in the last 3 years. Documentation for meeting all three criteria is required. Potential residents must be enrolled in the Coordinated Entry System through South Dakota Housing as referrals are determined by South Dakota Housing requirements. The Coordinated Entry Assessment phone number is (605) 665-3401.
Each resident at Cedar Village enjoys the comfort of a single-bedroom unit equipped with a living room, dining area, and a convenient kitchenette. Our facility is designed to minimize issues related to patient density while providing an environment for group services.
Lewis & Clark Behavioral Health Services provides care for approximately 1,400 adults coping with severe and persistent mental illness. Historically, these individuals had received care at the Human Services Center, but in recent years, LCBHS has taken on a larger role in providing community-based care. Many of these individuals have resided in low-cost or subsidized community-based housing. Recognizing the unique needs of this group, LCBHS staff has long understood that the typical housing arrangement falls short of meeting their needs. To address this, LCBHS has focused on creating housing options that incorporate assistance with activities of daily living, ensuring that no one falls through the cracks in our care.
