Systems of Care

South Dakota communities have a wealth of resources designed for the well-being and success of our youth, but with varying requirements it can be challenging to effectively navigate them all. In response Lewis & Clark Behavioral Health Services utilizes a Systems of Care – Care Coordinator to collaborate with schools and families, facilitating access to tailored resources and support for youth, with the aim of enhancing each student’s path to success by addressing their unique challenges, aspirations, and goals through personalized action plans.

Understanding the Roles of Your Care Coordinator

Initially your Care Coordinator will get to know you, your family, your strengths and needs, as well as answer any questions about the SOC process. Your Care Coordinator will respect you, your opinions and your goals for your child. Your Care Coordinator will request paperwork giving them permission to talk to other resources that can help develop your action plan. Initially, there will be weekly contact, in-person or by phone, to include you in each step. Team meetings may be arranged as needed and will include all the important people in you and your child�s life to better develop an action plan uniquely for your child and family.
